Thalamic Pain Syndrome

Thalamic pain syndrome is a central post-stroke pain disorder resulting from thalamic infarction. Clinical features include contralateral dysesthesia, allodynia, and chronic neuropathic pain. Pathophysiology involves thalamocortical pathway disruption and central sensitization. Diagnosis is clinical, supported by imaging demonstrating thalamic lesions. Management includes neuropathic pain medications, neuromodulation, and rehabilitation; prognosis is chronic with variable pain control.
