Tabes Dorsalis

Tabes dorsalis is a late manifestation of neurosyphilis affecting the dorsal columns of the spinal cord. Clinical features include sensory ataxia, lightning pains, and loss of reflexes. Pathophysiology involves chronic inflammatory demyelination and neuronal degeneration. Diagnosis is based on serologic tests for syphilis, CSF analysis, and clinical findings. Management includes penicillin therapy and symptomatic care; prognosis is variable, with residual neurological deficits common.
