Lewy Body Disease

Lewy body disease encompasses disorders characterized by alpha-synuclein aggregation in neurons, including Parkinson disease dementia and dementia with Lewy bodies. Clinical features include fluctuating cognition, visual hallucinations, parkinsonism, and REM sleep behavior disorder. Pathophysiology involves progressive synucleinopathy affecting cortical and subcortical networks. Diagnosis is clinical, supported by neuroimaging and biomarkers. Management includes cholinesterase inhibitors for cognitive symptoms, cautious use of dopaminergic therapy, and supportive care; prognosis is progressive with eventual functional decline.
