Adrenal Myeloneuropathy

Adrenal myeloneuropathy is a rare disorder characterized by concomitant adrenal insufficiency and progressive myeloneuropathic features, often associated with metabolic or autoimmune etiologies. Clinical manifestations include limb weakness, spasticity, sensory disturbances, and autonomic dysfunction. Laboratory evaluation reveals adrenal hormone deficiency, and neuroimaging or electrophysiologic studies may demonstrate spinal cord and peripheral nerve involvement. Management is twofold: hormone replacement therapy for adrenal insufficiency and supportive care including physical therapy for neurological deficits. Prognosis depends on timely recognition and treatment of endocrine dysfunction, although neurological deficits may be progressive.
